This program funds state offices to strengthen rural health care delivery systems by maintaining a focal point for rural health. It supports linking small rural communities with state and federal resources to develop long-term solutions that improve access and quality of care for nearly 60 million rural residents.
Eligible applicants include state governments and others as specified in additional eligibility information. The program targets entities involved in rural health care delivery.
