This program supports organizations with statewide or tribal reach to embed early childhood development experts in pediatric practices serving Medicaid/CHIP families. It funds team-based care delivery focusing on mental health, housing, nutrition, and child development screening, caregiver education, and referrals. Recipients will also build pediatric staff skills statewide and improve policies to sustain high-quality pediatric primary care, aiming for measurable improvements in service delivery and child development outcomes.
Eligible applicants include Native American tribal governments and organizations, small businesses, state and local governments, nonprofits (with or without 501(c)(3) status), and institutions of higher education. Eligibility is broad, covering various public and private entities.
