This funding supports projects that implement health-related social needs screeners with community members, facilitate referrals to community programs addressing those needs, and track the usage of services. The focus is on leveraging Community Health Workers to improve health outcomes in high-priority census areas by addressing social determinants of health such as cardiovascular risks and other community health factors.
Eligible applicants are organizations employing Community Health Workers serving patients or residents in high-priority census areas.
