This opportunity funds current or new Diabetes Self-Management Education and Support (DSMES) sites to achieve ADA recognition or ADCES accreditation. It provides technical assistance and support to help sites become independent or operate under the MT DEAP Umbrella, enhancing diabetes care quality.
Eligible applicants are new or current DSMES programs aiming to achieve ADA recognition or ADCES accreditation. Both independent sites and those under the MT DEAP Umbrella may apply.
