This program offers subawards to healthcare facilities working with asthma patients to implement quality improvement projects based on Expert Panel Report-3 Asthma Guidelines. Facilities receive funding and professional support including EHR optimization, educational presentations, scholarships, and patient materials. Additional funding is available for tobacco screening/cessation components. Participation is voluntary and applications are submitted online.
Eligible applicants include healthcare clinics, tribal health, community health centers, school-based health centers, and other healthcare facilities working with asthma patients.
