This formula-based grant supports service delivery to individuals with Serious Mental Illness and/or co-occurring substance use disorders experiencing homelessness or at imminent risk. Funded activities include street outreach, case management, and services not supported by mainstream mental health programs, targeting nonprofits and public agencies in Arizona.
Eligible applicants are nonprofit organizations and public agencies located in Arizona. The grant targets organizations serving individuals with serious mental illness and substance use disorders who are homeless or at risk.
